I was able to reproduce gpt-5-alpha in Cursor by directly adding it as a custom model. However, I discovered that any model name containing the string "gpt-5" displays the thinking model icon (brain icon) and allows Max Mode to be enabled, whereas other custom models I added (e.g., gpt-6-alpha, claude-5-opus) do not show this behavior. Currently, it is not usable.
